Anaplastic astrocytoma Word listening - Heschl gyrus Brain:   Postoperative changes of partial biopsy of mid left temporal lobe mass noted. The bulk of the tumor mass remains measuring 3.6 x 3.2 x 3.2 cm. Solid and cystic components present. The margins are well-defined. No appreciable enhancement.   The mass involves the middle and inferior left temporal lobe gyri. The superior gyrus of the left temporal lobe is spared.     Diffusion tensor imaging:   The left mid temporal lobe tumor extends and deep just abutting the periventricular white matter coursing anteroposteriorly.   Functional MRI:   Auditory: Auditory activation seen on word listening task in the superior temporal gyrus / sylvian fissure as expected bilaterally.   Language:   Language lateralizes to the left cerebral hemisphere.   Verb generation demonstrates activation in left Wernicke's area within the superior temporal sulcus (series 112, image 7). Activation also seen in the left supramarginal and angular gyri ( series 112, image 6 and 4).   Word listening demonstrates stronger activation in the left Wernicke's area within the superior temporal sulcus (series 106, image 4).   Motor:   Finger tapping demonstrates motor activation as expected in the central sulcus and postcentral sulcus bilaterally.     Impression:   Language lateralization to the left cerebral hemisphere.   Left temporal lobe tumor centered in the middle and inferior gyri. Activation in Wernicke's area seen just superior in the superior left temporal sulcus as expected.
