Presentation

Patient presents with fever, headache, and nasal congestion.

Clinical differential diagnosis

Concern for sinus disease.

Findings

Sinus CT:

There are air-fluid levels within the maxillary and sphenoid sinuses as well as opacification (mucosal thickening and fluid) of the frontal and ethmoid sinuses.

Although these findings are not specific to acute sinusitis, given the correct clinical picture, acute sinusitis can be suggested.

Treatment

If there is concern for obstruction, functional endoscopic sinus surgery (FESS) may be considered.

Diagnosis

Sinus air-fluid levels which may represent acute sinusitis.
