CASE: Chronic PE


EXAM QUALITY:

HIGH

  1. Can clearly see to the proximal subsegmental level
  2. ROI on main PA = 331 HU. (ROI 300 HU is often high quality if no/minimal limitation.) 
  3. LIMITATIONS: None

PE FINDINGS:

CHRONIC PE, right-sided

  • RLL lobar level (also see on coronal)
  • Thin web-like filling defect is a chronic PE feature
  • It is important to look at prior exam to see if present previously. This has decreased since prior CT PE exam performed 3 months earlier (image below):

ANCILLARY FINDINGS:

No sign of RV strain.

IMPRESSION:

Chronic right-sided pulmonary embolism most proximally involving the lobar level in the right lower lobe. No sign of right ventricular strain. RV/LV ratio is < 1, normal.
